From Tuesday 14th November 2023, patients registered with NHMC, who have Universal Care Plans, we will able to view their Care Plans on the NHS App.  

We encourage these patients to access their Care Plans via the NHS App and then feedback their experience using this form Viewing your Universal Care Plan via the NHS App (office.com).
